About the role
Join our Enhanced Care Management team to help individuals with complex needs live better at home. You will be the main contact for clients, coordinating their care across various providers and ensuring timely access to necessary services. This hybrid role involves significant travel within Los Angeles County.

What you'll do
- Evaluate client needs across physical, mental, and social health domains
- Create and manage client care plans and goal settings
- Deliver services in various settings, including in-office, telehealth, or field-based
- Connect clients to relevant social services and support systems
- Advocate for clients with healthcare professionals
- Apply evidence-based methods like Motivational Interviewing and Trauma-Informed Care
- Conduct outreach to enroll clients in the ECM program and log activities in the CRM
- Monitor client progress and update SMART goals
- Promote mental health awareness
- Arrange transportation services
- Complete all required documentation and outcome measures within set timeframes
- Maintain current patient health records in the EMR and other business systems
- Submit monthly reports to ensure program adherence
- Participate in assigned training sessions
Requirements
- Active, unrestricted ACSW, LCSW, LMFT, or LPCC license in California
- 1+ year of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- Available to work Monday-Friday, 8:30am-5:00pm, with potential for evening and weekend flexibility
- Familiarity with government and community resources related to social determinants of health
- Strong verbal and written communication abilities
- Positive interpersonal skills
- Valid driver's license, clean driving record, and reliable transportation
- Basic computer proficiency, including Google Workspace, MS Office, and internet usage
