About the role
Vynca is looking for a Lead Care Manager to join our Enhanced Care Management team. You will serve as the primary point of contact for clients, coordinating between their medical providers, pharmacists, and social support networks to ensure high-quality care.

What you'll do
- Perform needs assessments covering physical and mental health, substance use, housing, and palliative care.
- Create and manage personalized care plans with specific goals.
- Meet with clients in their homes, at medical offices, or via telehealth.
- Advocate for clients during interactions with primary care physicians and specialists.
- Facilitate connections to community resources and transportation services.
- Document all interactions and progress in our electronic medical record and CRM systems.
- Conduct outreach to engage new members in the program.
- Prepare monthly reports to maintain program compliance.
Requirements
- Minimum of 2 years of experience as a care manager, care navigator, or community health worker.
- Ability to work Monday through Friday, 8:30am to 5:00pm PT, with occasional evening or weekend flexibility.
- Must reside within 20 miles of the Santa Ana territory to accommodate daily travel.
- Valid driver license, clean driving record, and reliable personal transportation.
- Familiarity with social determinants of health and local community resources.
- Proficiency in Google Workspace, MS Office, and general computer navigation.

Practical notes
This is a hybrid role requiring up to 5 days of travel per week within Orange County. Employment is contingent upon a background check and drug screening.
