About the role
This position leads Form Health's national telehealth clinic, overseeing clinical operations for a large, distributed medical practice. You will ensure high standards of care, patient safety, and a positive environment for the clinical team. The role focuses on strategic and operational leadership, working closely with various departments to achieve performance goals.

What you'll do
- Oversee and grow clinical operations, including physicians, advanced practice providers, dietitians, and support staff, to deliver excellent, evidence-based care and a superior patient experience.
- Manage clinical operational performance, including service level agreements, capacity, utilization, and workforce planning across multiple states, in collaboration with Practice Operations and Finance.
- Develop clinical operations strategy and drive practice improvements, such as physician-APP models, new service offerings, and workflow enhancements, in conjunction with the COO, CMO, and Executive Leadership.
- Provide strategic input to Product and Engineering teams on tools and features affecting clinicians and patients, ensuring alignment between the product roadmap and evolving care delivery models.
- Lead, mentor, and develop clinical leaders and managers, establishing scalable leadership structures and supporting clinician well-being.
- Represent the clinical organization externally at conferences and client meetings, and assist Sales and Marketing with clinical messaging.
- Maintain a small patient panel for ongoing clinical practice.
Requirements
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board certification in Internal Medicine, Family Medicine, Endocrinology, or a related subspecialty, with an active and unrestricted medical license.
- American Board of Obesity Medicine (ABOM) certification, or a strong commitment and eligibility to obtain ABOM certification within a specified timeframe after joining.
- A minimum of 5 years of clinical practice experience, with significant exposure to obesity medicine and chronic metabolic disease management.
- At least 3 years of clinical leadership experience within a scaled, multi-state telehealth setting or a large virtual/hybrid practice, demonstrating experience managing large provider teams and multi-state operations.
- Demonstrated ability to influence cross-functional teams and drive operational performance and clinical quality.
- Strong people leadership skills within a multidisciplinary team, with a history of developing clinical leaders and a commitment to team culture, clinician well-being, equity, diversity, and inclusion.

- Compensation includes a competitive base salary ($275,000 - $300,000 USD) plus an annual leadership bonus and stock options.
