Form Health is expanding its care offerings with a new diabetes program. We are looking for a skilled Registered Dietitian to join our team and provide specialized nutrition support to patients managing diabetes. This role is central to our mission of delivering high-quality, empathetic care through our virtual platform.

Key facts

What you'll do

Deliver personalized medical nutrition therapy to patients via video consultations.

Build supportive, non-judgmental relationships with patients to guide their health journey.

Develop tailored nutrition strategies considering individual patient needs.

Educate and counsel patients on diabetes self-management, healthy eating habits, and behavioral changes.

Interpret glucose monitoring data, including from Continuous Glucose Monitors (CGMs).

Assist patients with managing co-existing conditions like Chronic Kidney Disease and cardiovascular issues.

Provide feedback on patient food logs and daily photo submissions.

Offer ongoing support to patients through text messaging within the Form Health application.

Contribute to the development and enhancement of our cardiometabolic services.

Requirements

Registered Dietitian with 3-5 years of experience in patient counseling and behavior modification techniques.

Demonstrated experience working with individuals managing diabetes, particularly Type 2.

Experience with medical weight loss programs, patients using weight loss medications, meal replacement plans, and cognitive-behavioral therapy.

Nice to have

Certified Diabetes Care and Education Specialist (CDCES).

Skills & tools

Telehealth platform usage.

Mobile application communication.

Practical notes

Salary range: $70,000 - $80,000 USD annually.
