What you'll do

• Manage a varied patient caseload, ensuring effective care coordination and quality.
• Guide and oversee Home Health Aides to maintain high patient care and clinical standards.
• Create and implement personalized care plans based on patient assessments.
• Coordinate care with Primary Care Physicians, therapy providers, and internal teams.
• Educate patients and families about their health needs and treatment plans.
• Maintain accurate and timely electronic patient records, aiming for real-time documentation.
• Ensure all documentation meets regulatory and company standards within 24 hours.
• Communicate effectively with external and internal healthcare partners.
• Escalate patient concerns and order management issues to Clinical Leads promptly.
• Participate in an on-call rotation, including approximately one weekend per month and holidays, with potential weekday phone triage.
• Strive for high caregiver satisfaction through compassionate care.
• Monitor and work to reduce patient readmission rates.
• Ensure extended visits are within acceptable limits and administrative tasks like mileage reimbursements are completed.

Requirements

• Associate's Degree or Baccalaureate School of Nursing Degree.
• Current and valid Registered Nurse license in Massachusetts.
• Two years of experience as a Registered Nurse in a clinical setting.
• Reliable transportation, a valid driver's license, and current auto insurance.
• Ability to work flexible hours, travel locally, and provide on-call services.

Nice to have

• Bilingual in Spanish.
• One year of home health experience.
• Case management or supervisory experience.
• Pediatric or adult care experience.
• Knowledge of Medicaid paid family caregiving programs and HHA scope of practice (training provided).
• Familiarity with OASIS (training provided).
