Hospital Relationship Manager

About the role

This position is central to establishing abby-care's presence within hospitals. You will act as the primary contact, educating clinical teams about our family-led care platform and facilitating patient transitions from hospital to home. This is a field-based role focused on direct engagement within hospital settings.

What you'll do

• Maintain a consistent, visible presence within assigned hospitals, identifying key staff members like discharge planners and nurses.
• Educate hospital personnel on abby-care's services through presentations and informal discussions to identify suitable patients.
• Generate a steady flow of qualified patient referrals, meeting specific volume targets.
• Collaborate with hospital staff and abby-care's intake team to ensure smooth patient admissions from hospital to home care.
• Document all facility interactions, track relationship development, manage your referral pipeline, and analyze growth metrics using our CRM.

Requirements

• 4+ years of experience in a hospital or healthcare environment.
• Demonstrated history of building relationships with diverse hospital teams, including discharge planners, social workers, and nursing staff.
• Experience working directly within hospital floors, nursing stations, and discharge planning areas.
• Ability to become a trusted resource for clinical teams, assisting with patient placement and transition challenges.
• Deep understanding of clinical environments and patient transition processes.
