• Maintain a consistent presence within assigned hospitals, identifying key personnel like discharge planners, social workers, and nurses.
• Educate hospital staff on Abby Care's programs through presentations and informal discussions to identify suitable patients.
• Actively seek eligible patients and develop a steady pipeline of referrals, with performance tied to volume metrics.
• Collaborate with hospital teams and Abby Care's intake department to ensure efficient patient admissions from hospital to home.
• Document all facility activities, relationship progress, and pipeline status using a CRM to track growth.

Requirements

• At least 4 years of experience in a hospital or healthcare setting.
• Proven ability to build relationships with various hospital teams, including discharge planners, social workers, and nursing staff.
• Experience working directly within hospital environments, such as patient floors, nursing stations, and discharge planning areas.
• Demonstrated ability to become an essential resource for clinical teams, assisting with patient placement and transitions.
• Strong understanding of hospital workflows, discharge planning processes, and patient transitions to home care.
