We are seeking a dedicated Case Management Coordinator for our ODP program, who will play a pivotal role in managing the pre-admission process and fostering essential relationships within the community. This position is integral to our operational team, leveraging your knowledge of the Office of Developmental Programs (ODP) and the Pennsylvania waiver system to enhance our care management services. You will contribute strategically to our mission of expanding our community presence and improving the quality of care provided to individuals.

What you'll do
- Oversee a caseload of individuals enrolled in the ODP across Eastern and Central Pennsylvania, ensuring they receive top-notch care tailored to their needs.
- Provide guidance and mentorship to Direct Support Professionals (DSPs) to ensure compliance with ODP standards, with support from the HR team.
- Collaborate with the Case Management Team to facilitate new Starts of Care within a designated geographic area, ensuring smooth transitions.
- Ensure that Individual Support Plans (ISPs) are effectively implemented, maintaining updated records and documentation, and preparing quarterly reports.
- Manage staff schedules for your caseload, ensuring that electronic documentation is completed accurately and on time.
- Conduct weekly audits of schedules to support pre-billing teams and ensure compliance with ODP regulations.
- Educate families about service definitions and compliance requirements to empower them in their care journey.
- Lead introductory meetings with families and Support Coordinators (SCs) to present the Abby Care model and establish rapport.
- Serve as a liaison for pre-admissions, coordinating with intake and HR teams, and managing communications through HCSIS and AE.
- Build and maintain relationships with leadership from Support Coordination Organizations (SCOs) and individual Support Coordinators.
- Identify opportunities for market expansion by engaging with community organizations and advocacy groups to enhance service delivery.
- Update the Payroll/Pre-Billing team on documentation and Electronic Visit Verification (EVV) records to ensure compliance with ODP standards.
- Provide training and mentorship to the Care Coordination team, ensuring adherence to ODP standards and best practices.
- Act as the primary escalation point for Community Care Coordinators regarding incidents, complex issues, or billing inquiries.
- Conduct quarterly reviews of documentation, EVV, and service notes to ensure compliance with ODP 6100 regulations.
- Oversee the Enterprise Incident Management (EIM) process, ensuring accurate and timely reporting to state authorities.
- Track and manage incoming complaints for documentation and communication with AE.
- Participate in the development and implementation of Quality Assurance and Incident Management audits, as well as quarterly Quality Management Planning meetings.
- Relay high-level updates regarding regional performance, systemic risks, or trends to the ODP Lead.
- Collaborate with Community Event Associates to support the planning and execution of regional community events.
Requirements
- A minimum of 2 years of experience in a role related to the Office of Developmental Programs (ODP), such as Support Coordination, Program Management, or Residential Director, or 4 years of diverse experience in the ODP field.
- At least 2 years of direct experience working within the Pennsylvania ODP framework.
- Comprehensive understanding of ODP waiver regulations, HCSIS, EVV, PROMISe, and the ISP process.
- Familiarity with relationships involving SCOs, documentation systems (EMR/EHR), and EVV requirements.
- A valid driver's license and access to reliable transportation are essential.
- Proficient in speaking, reading, writing, and understanding English.
- Ability to perform physical tasks necessary for in-home visits, including occasional lifting or pulling of up to 20 lbs.
- Capable of navigating physical environments, such as stairs, during home visits.
Nice to have
- Previous experience in Case Management is advantageous.
- Established connections with County MH/ID offices and SCOs in Eastern Pennsylvania.
- Bilingual capabilities are a plus.
- Willingness to work outside of standard business hours when required.
